Food Permit Submission

Please email your signed and completed food permit to dbarge@marchofdimes.org. Food Permits are due to the city by Wednesday, October 11, any permits turned in late will result in March of Dimes paying an excessive expedite fee. Please help us save money and submit your form on time and make sure your permit is completely filled out before submitting.

Menu Selection

Menu selection, supply needs, and completed permit are due MONDAY, OCT 9

To ensure there is no overlap in menu, all menu selections will be reviewed. PRIORITY FOR MENUS WILL BE ON A FIRST COME, FIRST SERVE BASIS - the first to submit their menu gets the first choice in what they serve; this is to eliminate too much overlap in main ingredients.

Please be aware that if there is a duplication in the main ingredient we will ask for a second option
We encourage your portions to be slightly larger than a normal bite size tasting portions.
Chefs will be notified of any changes recommended to their menu selection individually if duplicates arise, final changes will be requested by October 10.
No desserts, please – plated dessert will be provided during the Live Auction portion of the evening.
March of Dimes will contact you by Thursday, October 12 if there are duplicates and help determine selection.
